- The Federal Polytechnic, Offa, was established in 1992 after a presidential announcement by Military President Ibrahim Babangida during a 1991 state visit.
- The announcement occurred at the Palace of the late Oba Mustapha Olawoore Olanipekun Ariwajoye II, the Olofa of Offa.
- A Local Task Force, led by Alhaji Tiamiyu Olatinwo and comprising six other Offa indigenes, oversaw the establishment process, with Chief Ayotunde Raji as Secretary.
- Engr. (Mrs.) Taiwo Adeife Osemeikhian served as the pioneer Rector from 1992 to 2000.
- The Federal Task Force, supervised by the Director of Science and Technology, Federal Ministry of Education, made major policy decisions and acted as the Governing Council.
- Dr. Razaq Bello succeeded Engr. (Mrs.) Taiwo Adeife Osemeikhian as Rector from 2000 to 2006.
- Dr. Mufutau Olatinwo became Acting Rector in 2006 and later the third substantive Rector since 2008.
- The Polytechnic saw its first Governing Council established in 2000, followed by subsequent councils in 2005 and 2009.
- The fourth and current council, chaired by Chief Okey Ezenwa, was inaugurated in April 2013.
